Hi there in my experience. I send the I-693 filled out by the civil surgeon and sealed in an envelope and a copy of my vaccination worksheet from the my county or origin. And I didnt do another medical examination, I just completed the vaccination requirement part which is MMR, tetanus, HPV (for women 9-26 years of age) and varicella titer. Here's a civil surgeon locator https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV and hope you found one that will give you a lower price if she didnt completed her vaccination and for the civil surgeon to complete the form I-693 part 2 section 5 and part 5 and dont forget the Part 1 of it to be completed by you. GOod luck

Hi, You can check my timeline.. I didn't have any vaccination nor interview.. I would suggest that you just wait for instruction/letter from the USCIS if your wife needs it. some cases they don't need it so to save money better wait for it, who knows, your wife will have the same case like me...or check the Civil Surgeon on this site if you need it..usually it will cost you less than $50.

Hi..I got this info again from USCIS :

http://www.uscis.gov/portal/site/uscis/men...0004718190aRCRD

Modifies Report of Medical Examination and Vaccination Record

WASHINGTON—U.S. Citizenship and Immigration Services (USCIS) announced today that it has revised Form I-693, Report of Medical Examination and Vaccination Record. The revision was necessitated by changes to the Tuberculosis (TB) Component of the Centers for Disease Control and Prevention’s Technical Instructions for Civil Surgeons.

This revised form (edition date 04/02/08) must be used for any medical examination completed on or after May 1, 2008. Previous editions of the Form I-693 may not be used on or after that date.

The medical examination Form I-693 provides USCIS results of a medical examination for applicants filing for adjustment of status to become permanent residents. The examination is required to ensure that an applicant is not inadmissible to the United States on public health grounds.

The new form lists seven TB classifications at the bottom of its first page. Civil surgeons must record the results of all medical examinations conducted on or after May 1, 2008, on the new form.

Additionally, the current vaccination supplement will not be accepted for any vaccination assessment completed on or after May 1, 2008. An updated vaccination supplement has been included in part 2 of the revised form.

The revised TB Component of the Technical Instructions is available for review in the "Related Links" section of this page. For more information on the revised Form I-693, call the National Customer Service Center at (800) 375-5283.

I'm still suggesting to wait for their instruction before you go on with it..So you spend too much if your wife will have the same case like me..

best regards..

Hello,

I am a bit lost about the new med requirements. I had my med examination in France in June 2008, so the new requirements were not applied yet.

When I heard about them, I had in France the vaccinations I didn't have, a second MMR shot and Gardasil HPV. I also asked my doctor to have the TB test done, the new TST that CDC requires. So basically, I have eveything ok for the new requirements.

However, I had those done by my family doctor in France. Do you think that they will want me to retake those in the US when I go see the civil surgeon before my AOS? I read that when you had the med examination within a year before AOS, another med exam isn't required in the US, however part 1, the vaccination section of part 2 and part 5 of I-693 have to be filled by the USCIS surgeon in the US. I got that.

But TB test has to be filled out at the beginning of part 2 of I-693, as a part of the total med examination. So maybe the civil surgeon will refuse to fill out this part for TB. My family doctor in France translated the results of TB test on a paper. If I give a copy of that along with what the USCIS surgeon will fill on I-693, do you think I will be fine??

I heard that you have to also give the results of the Xrays. However, I had the Xrays done in France for the med examination, and so I guess the results are in the package I will be supposed to give at the POE (the scelled envelop I got back from the Embassy for the K1). Do I also have to give along with AOS application a copy of the results of Xrays?

Thanks a lot!!

Hi there! I think that the new requirements for medical examination will take effect on the specific date which is or or after june 1, 2008. If you have your medical examination done in france on or after june 1, 2008, the medical center connected with the us embassy in france should've done those new requirements. For further information please look at this USCIS site with regards to the new medical examination and vaccination requirement.

Hello,

Yes I know that they should have done the new requirements as I had my med examination on june 6th. But they didn't tell me anything and the doctor didn't know. So as soon as I figured that out, I had the shots or tests that was missing done by my family doctor, who isn't a USCIS one. But I mean, I have everything required done now. I just have no idea how to tell to USCIS that. Because if I don't go through the all examination again, the doctor in the US will refuse probably to fill out the TB part on I-693. See what I mean?? Of course he will complete the vaccination part that he is supposed to do, and then what if he refuses to also complete the TB part? I will take all the results I got after my med examination in France, and hopefully this will be enough...

I am concerned because it seems like even in the US, many doctors have no idea about what they are really supposed to do. Most of them have no clue that a K1 who had the med exam overseas within a year doesn't need to have another one for AOS...

Hi. You do the I-693, not the I-693A.

Great link that explains this stuff pretty clearly... what's necessary in terms of vaccines and TB test requirement since the form has changed (all new requirements re: the test and vaccines went into effect 8/1/08!).

http://www.uscis.gov/portal/site/uscis/men...0004718190aRCRD

The I-693 forms were revised on 06/05/08. New info. Directions to the new I-693 link below.

http://www.uscis.gov/files/form/I-693instr.pdf

Here's the form itself.

All you have to do is the vaccination part (you already did your medical in France and it was accepted; it was done before 8/1/08). See my answer to the above person.

An approved embassy doctor should have done a form for the medical that you submitted to the Embassy during your interview and then you handed those papers off at the POE. Now, the only thing that you need to do is the vaccination part, the transcription. See notes above from me to another poster. Again, you did your medical before 8/1/08 (when the new requirements went into effect), so you shouldn't have to redo all of it, nor should you have to redo the TB test, as your medical was done in June.

The only thing that you should have to do is the vaccination part. You'll have to ask your doctor if you will need all the new vaccinations or not... that's a good question. I would think not, but the Civil Surgeon should know.

Good luck!

I used I-693 FORM according to my civil surgeon. He has a complete guidelines on how to accomplish the form. I followed all his recommendations and I've got no problems about it. Why wait for another letter from USCIS? it will just take so much time. Having an RFE means you're not doing it right. I spent more than $200.00 for all the required tests and vaccination. I don't have regrets about it. I received my green card even without interview. GOOD LUCK TO YOUR AOS PROCESSING!!!! :star:

Faustine

Hi there! I think that the new requirements for medical

examination will take effect on the specific date which is or or after june 1, 2008.

Hi there. For me, since you have done your medical examination in france and they dont know about the new requirements then its not your fault, its theirs. But I understand what youre saying that it will be you who will be affected, what if you tried to go back to where you have your medical done and tell them your situation? Im sure by this time they already know about the new requirements. Either way I still think that its not your fault and yes youre right most of the civil surgeon here in the states would want for you to have all the process done with them like the medical examination and vaccination EVENTHOUGH THEY KNOW THAT WE'RE K1--MOSTLY ITS MONEY BUSINESS  But mind you not everyone and there will still be some civil surgeon who would just do the vaccination alone--I found one. In your case I would suggest if you can contact the personnel where you had your medical done if not then just bring all the paperworks with you showing that you have done the tb testing, x-ray etc, so that when you come in here you can explain to the civil surgeon your case. Good luck and blessings
