hi folks!

i'm based here in ca... and i went to the doctor this afternoon first, to complete my vaccination ( i still lacked one tetanus shot) and second, to ask the doc to fill out the 1-693 form which will form part of the docs i need to submit for aos. (i did bring to the clinic ds 3025 issued by the hospital back home where i had my medical)

it was a bit confusing coz the doctor initially refused to sign part 5, arguing that he didnt conduct the full medical, and therefore could not sign the certification part. i showed him the i-693 instructions, stipulating that k3 visa holders filing aos within one year of overseas medical exam dont need full medical, but we do need a civil surgeon to complete the vaccination part of part 2 and to sign part 5 or the certification.

he finally signed part five but he edited the text, putting instead, completed tetanus injection, in place of examination.

would this create a problem for me?

should i seek another doctor to fill out the i-693 before sending the package?

would appreciate your two cents worth on the topic.

btw, i was charged $140 for the entire thing, one tetanus shot and filling out form i-693..

If he put everything in a sealed envelope I'd just go ahead and submit it with the AOS. Put a copy of the DS 3025 also.

I'd submit as soon as you can because vaccination requirements will change after August 1, 2008.

Hi there! Thats not surprising that most civil surgeons won't sign the part 5 of I-693 unless they will do the whole thing, the medical examination and vaccination. (well it will generate money for them than just to do the vaccination alone :secret: And they have different pricing! So better to call around numbers that the USCIS provided in the civil surgeon near in your area) Anways, I encountered alot of that when I'm trying to find a civil surgeon who would complete the shots that i lack. Luckily, I found one who advice me to just have my MMR and tetanus shot in a public clinic FOR FREE and we can just present to them the YELLOW CARD that they will give me in the public health clinic, so it save us like $120. But now that the USCIS had new regulations for vaccination requirements and one of those is to have HPV shot for women ages 9 to 26 and I just turned 26 so I'm included. So basically, we paid $170 for the HPV (gardisil) shot and I still need to do the varicella titer (eventhough I already had Varicella history) still they insisted its one of the new requirements oh well its $20 plus $20 for the civil surgeon to sign the form PART 2 and PART 5, ITS IMPORTANT FOR THEM TO SIGN THIS EVENTHOUGH THEY DID NOT DO THE MEDICAL EXAM. There's civil surgeon out there where you can find this deal and wont give a highprice and say that they wont do it not unless you do the WHOLE MEDICAL EXAM THROUGH THEM. Especially K1 BASED AOS, IT CLEARLY STATES IN INSTRUCTIONS FOR I-485 THAT WE DO NOT HAVE TO REDO THE MEDICAL EXAM PROVIDED THAT WE HAVE DONE IT WITHIN 1 YEAR DURING THE APPLICATION FOR AOS. Please do make further reading with regards to this. Plus you can call USCIS customer service center. Good luck to all of us. bless us ^_^

Hello!

I had the same problem.. Called ALL the doctors on USCIS list in my area and NONE of them agreed sign the form without doing the full examination.. So I am just going to show up the day of my appointment with the USCIS instructions and I will show them.

Oh yeah, and I also called USCIS to ask them if it was normal that authorized doctors refused such thing regardless to USCIS rules. They didn't care at all, saying that doctors should know the legislation (that they obviously don't) and that anyway, they cannot call every doctor to force them to do what they are supposed to. So pretty: It is not our problem, we don't care, deal with your own problems without us.

Hi there,

My experiences sucked as yours: I called many CS in my area and they would tell me I should do the whole medical examination again along with the vaccinations transcriptions in order for them to sign the form. Of course, the prices were pretty high and they were very rude to me on the phone questionning me about how the process should be conducted.

Well, I called one last CS and they said I didn't have to take the whole medical and that they'd only charge me $15 for the vaccination part. Also, I asked them whether I needed to translate my report into English and they said no because they were bilingual. Well, I showep up last Friday and everything went perfect! Since I had al the shots, they gave me the sealed envelope and they didn't even charge me for transcribing my records into English. So... yay for them!

My husband and I will send my AOS package tomorrow. Hope the rest of the process goes smoothly!
