I-751 questions about affidavit

hello everyone!

hi...am a non -US citizen and am gonna be applying for removing conditions next week and i just have a few questions;

am wondering where do we get the sworn affidavit forms? my husband argued me about it....he says affidavit means its a legal form of documents and needs to be notarized....i dont know,i just thought of that as a letter from your family or friends swearing that the marriage is legit...and on it they stated thier names and signed it and maybe their address and how they achieve the knowledge about us...(in other hand,their relation to us)...thats what i thought it is....but now am so confused,he wants me to search a documents on USCIS, i tried i couldnt find one( probably bcoz i dont know what am looking for,lol...) so i dont know....

hope somebody here can help me with this...give me a clue on GENERAL INSTRUCTION NO.5

what is that? is it just a letter or do we have to print some kind of form thats from USCIS...and have it signed by our family and friends.

The below link is a template that you can follow, add your information, print and YES have notorized however keep in mind that they advise you to have this done only if you have insufficient proof that your marriage is legit. I did not send one in my particular case since my husband and I have financial accounts together, a son, travel records, pictures and insurance information however your case might be different.
