How do I Get.....

Hi. I'm getting my AOS package ready to send out and the last thing I have to do on my list is find out whether or not I need to send in a "Vaccination Suppliment". If so, where do I retrieve such information?

The Vaccination Supplement question comes up a lot and you will get conflicting answers from almost everyone.

For my AOS process, I only sent in the Vaccination Worksheet (which I got at my K-1 medical exam in Canada) and so I did the same when I sent in my daughter's AOS application later.

She got a letter in the mail stating that she needed the Vaccination Supplement, so I had it done in Seattle, and we took it to her AOS interview. It was not asked for, so I got it done for nothing.

It's your decision as to whether you want to get one done (at a civil surgeon here in the U.S.) or not, but if I had to do it over again, I would just go with the Worksheet.

I personally think if you have all your shots up to date, you can just send in the worksheet. I got the supplement done because i really didnt want to risk an RFE. If you go to the USCIS website, there is a whole list of civil sugeons you can go to to get the supplement done.

go to a civil surgeon and get the supplement. Why risk an RFE. It's not worth the trouble.

I actually just mailed in a copy of the front and back of my husbands shot record that he brought home with him.. He has paid for 3 doses of vaccines that he didnt need in Nigeria. I also sent proof of those .
