So I have a "real" name-- on my birth certificate and my passport and a nickname that I have always used (somehow I even managed to weasel it onto my credit cards and driver's license although I regret it now). On my 129-F documentation, I have been using the real name. However, the emails (proof of on-going relationship) and even some previous tax info and as I mentioned, my driver's license, all use my nickname. I put this nickname on my g325 under other names used. Do I need to include other explanatory materials in my 129F package? Like maybe a photocopy of my passport and driver's license demonstrating that even though the first names are different, the last names, the SS# and the face are the same?

I don't think it would hurt to add supporting documentation explaining your 'nickname'.

I'm often referred to as Kimberly even though my name is Kim. The Social Security office even had it wrong at one point. I also have no middle name but everyone seems to want to give me one. ;) Russell's given name is Te Ngere and that is the name that is on his documents but he goes by Russell so that is the name on our correspondence as well. I'm sure this is common enough.

You did note the name you're known by on the form already but if you're that concerned I dont see a problem adding an explanation and note it as an attachment on the form ..if only to put your mind at ease. ;)
