Currently we are waiting for out packet 4 to arrive. My fiancee has everything he needs for the interview and 2 weeks ago he sent the checklist IV-15 to the embassy. He sent it via Royal Mail with signature confirmation upon arrival, as of now it has yet to show up as arrived. He called the visa helpline yesterday, they told him there was nothing they could do to confirm it's arrival and that it's an issue with Royal mail..so they instructed him to go to website, download the IV-15 and resend it.

We have been looking all over the London embassy website and we can't seem to find the correct form! Could anyone help us find where on the embassy website it is located or could they send us a copy of it?

Thank you SO much for your help :) It's all rather frustrating!

I know it is quite frustrating! It took longer than I thought to received packet 3, and like you, they told me there were nothing they could do, except waiting!! I was quite angry! :angry:

For your case, I would try to post it on the England forum, maybe some people would advice you on that form...

Sorry that I cannot help you out! but who knows, maybe you will have an answer of them this week! :star:

Packet 4 from London was delivered for us 3 weeks to the day from when the checklist was sent in. That was June 14, so we are probably talking about a generally similiar processing time. I got all kinds of weird infornation from the Embassy, so don't go into too much panic yet. They said Packet 3 hadn't been processed. It arrived the next day with a postmark before the day she said it hadn't been processed. Go figure. I'd give it at least 3 weeks to receive Packet 4.

The USC can call the Dept of State and they can also tell things like if the P4 has been sent and what your interview date is. I knew our date about 3-4 days before P4 arrived. (202) 663-1225 Menu choices: 1, then 0. The people are very nice. Call back the next day if they say it's not scheduled yet.

This is close to the IV-15 http://www.usembassy.org.uk/cons_new/visa/iv/iv15v.html but doesn't look exactly like the one they mail you. The last page of the one they mail you asks for more information like phone, email, date of medical, date of wedding, case number. The checklist is pretty good, but I would write those other things in on the bottom if you have dates. And I would give it another week or call the Dept of State on Wed or Thursday.

There are scans of London Packet 3 posted on VJ by Capt Ewok, but they are out of date. Ours is dated April 2008 version.

I have scans of the new P3, but they are more than my personal album limit or I would post them to share with all. I tried. Didn't work.
