what vaccination form is that? and also the I-693A is that the same?

You will need the Tetanus shot here or what ever they call it.... Take the form you got from your Medical and have it transfered to a I-693A form by a Civil Surgeon... Hope this Helps... Dan

There is new form I-693 started from 4/2/08!!! (which is combined with I-693A)

AOS Process:

- 11-28-07 I-485/I-765/I-131 sent

- 12-06-07 NOA1 of AOS

- 12-18-07 received RFE for I-864

- 12-20-07 EAD and AP touched (Notice Returned as Undeliverable)

- 01-04-08 Biometrics appointment done

- 01-19-08 resent hubby's income proof

- 01-28-08 AOS touched (Response to RFE received, and case processing has resumed)

- 03-10-08 EAD and AP finally touched and approved!!! =)

- 03-12-08 Case Transfer to CSC

- 03-13-08 EAD Touched again (Card Production ordered)

- 03-17-08 AP and Transfer notice received in mail (AP valids until 03-09-09)

- 03-18-08 EAD touched again (Approval Notice sent)

- 03-20-08 EAD received in mail

- 03-27-08 AOS Touched everyday since 03-20 (Case pending)

- 04-01-08 Applied for my SSN with married name

- 04-07-08 SSN received in mail (YAY!!! No More ALIEN)

- 05-19-08 Received another RFE about medical

- 05-21-08 Visited family doctor for full medical exam

- 05-27-08 Visited civil surgeon to fill in I-693

- 05-28-08 Mailed out RFE reponse to CSC

- 05-30-08 USPS shows my RFE reponse was delivered at 11:22 AM in LAGUNA NIGUEL, CA 92607

- 06-04-08 RFE received and case processing has resumed

- 06-09-08 GC Card production ordered and welcome letter sent (6 months waiting!!!)

- 06-12-08 GC Approval Notice sent

- 06-16-08 GC and welcome letter received

Did you CS only fill out Part II of the vaccination session? Also were you required a TB skin test by the CS?


11-16-12: Sent I-130 for Dad.
11-19-12: Priority date/rec'd date.
02-06-13: Case approved, NOA2.
02-19-13: NVC received case.
03-08-13: Case # assigned--HCM2013******.
03-09-13: Paid AOS Fee $88.00.
03-09-13: Emailed DS-3032 from Beneficiary's email address.
03-13-13: AOS Fee showed as PAID & sent AOS package certified mail.
03-18-13: AOS package delivered @ 10:50 A.M.
03-18-13: DS-3032 accepted.
03-20-13: IV Bill Invoice.
03-21-13: Paid IV Fee $230.00.
03-23-13: IV Fee showed as PAID.
04-23-13: Sent IV package certified mail.
04-26-13: IV package delivered @ 11:13 A.M.

05-08-13: Case completed @ NVC per telephone call.

05-15-15: Case completed per email from NVC.

06-07-13: Rec'd email from NVC RE: interview date. (letter dated 06-05-13).

07-18-13: Interview date-- PASSED.

Yes my CS filled out everything for me.

Besides the TB skin test, I was required to have MMR and Varicella too, but luckily after the blood test, it shows that I was still immune, therefore, I don't need any extra shots. Just took the Td shot!!
