Question regarding evidence for I-751

We are getting ready to file I-751 in about a month and are busy putting the evidence together.

Regarding the Affidavits: Do they need to be notarized or is a letter with SS#, DOB, and signed by the person confirming our marriage enough?

Either notarized or with the following declaration at the end:

I declare under penalty of perjury that the foregoing is true and correct. Executed on (date).
