hey i have a question i am from illinois and i am living in japan right now i had to have my mom send me my birth certificate at first she sent me the short form which i know was wrong so i called her and asked her if she could go to the courthouse and get a copy of the long form but there is nothing on the back side of it i know it says that you need a copy front and back but if there is nothing written on the back is this the wrong birth certificate?

Dont quote me on this but i am sure if you have a passport thats valid for more than 5 years you dont need to give a copy of your birth cert. Maybe someone else can tell you more on this one. Good luck.

The long form usually includes more detailed information about your parents, including full names, address, occupations...etc.. it's also usually signed by the doctor who delivered you. It's an exact duplicate of the original. The short form is just confirmation that the longer record exists. ;) What you need is the long form and it doesn't always have a back to it. Mine did not. ;)
