I am Jamaican and my wife is a USC. We are putting the document together to file our I-130 petition. we want to send Affidavits to show that our relationship is bonifide. this is the issue... i saw an example of how this affiadvit is done on the website but we are wonder if we need to have the header

" BEFORE THE UNITED STATES CITIZENSHIP AND IMMIGRATION SERVICES

UNITED STATES DEPARTMENT OF HOMELAND SECURITY" for the person if the person doing it is jamaican. what header can we put there instead?

and the section that says "#A123456789"....is that the case number? if so...is it ok if i just leave that section blank or delete it?

i found the sample on this page...under "Lifting of Conditions (I-751) Related Forms"...then " Sworn Affidavit from Friends"

I was wondering the same thing. But would also like to know if the affidavit has to be notarized?

It does not state that it is a requirement.

Also can family members write to confirm marriage? And does it need to state that they are swearing under oath etc.?

Thanks in advance.

It doesn't have to be notarized and it can be from anybody who can say you have a bona fide relationship.

My best friend in Brazil wrote an affidavit for my husband and I when we sent out the I-129F. I mailed this in along with the translation.

I asked her to do this for us so she wrote what she wanted to - about our love and her relationship with us. It really was beautiful (L) .

I wrote one for my hubby. I did not need a header. I just wrote a regular letter and put a few photo copies of our wedding and dating years in the I-130 pack. No notary is needed. All the best.

Met Jan 1998, vows on 2006, Jay Jay born 2008, baby 2 - 2011

Look at time line for visa information
