14. DS-156 form � Nonimmigrant Visa Application (2 copies) and DS-156 K� Nonimmigrant Fiance(e) Visa Application (one copy).
� Every numbered part must be completed.
� Do not use dashes or the letters �N/A� or abbreviations.
Application must be presented by each fianc�(e) (K-1) visa applicant.
� K-2 applicants (children of K-1 applicants), K-3 and K-4 applicants (children of K-3applicants) complete only two
copies of form DS-156 � Nonimmigrant Visa Application.
� If the child is included on your passport, you should use your passport information to fill out the child�s application.

15. The fee for EACH for EACH K and V VISA APPLICANT (including children) is $131 payable in Ukrainian Hryvnya equivalent only. Beginning January 01, 2008 the fee for K and V visa can be paid only at the bank - the list of banks accepting payments can be downloaded from the Embassy website - it's within the Infromation Packet dodcuments.
16. Beginning August 25, 2003 the passports with the visas issued will be sent by the courier service to your address, the delivery and insurance cost is $8.

If not what's the waiting period? 7 to 10 working days. It is mailed through TNT. You must arrange for TNT to deliver to any of their offices near you in advance at http://www.ustraveldocs.com/ua/ when scheduling your interview. TNT does not deliver to the recipient, but only to their offices. You must then go pick it up from them. There is no extra charge for the delivery.

Per-person fee for a medical exam is $215 for adults and children. The Clinic performs medical examinations each working day (Monday through Friday), between 9:00 a.m. and 12:00 p.m. by appointment only. The results of the examination are given to the applicants at 3 p.m. on the same day. The fees for medical examination and vaccinations are paid in Ukrainian Hryvnya only.

Each applicant must present the following documentation:
International passport, 4 full frontal view 50mm x 50mm photographs, military service card (if applicible), vaccination chart with the seal of issuing clinic (mandatory for immigrants, optional for K visa applicants), and Embassy case number (As of 22 Apr 2013).

Estimated waiting time at the consulate on the day of interview? Varies. If there is a line of people outside the door, go directly to the front of the line. You are a US Citizen. US Citizens have the advantage and may jump to the front of the line ahead of non-US Citizens. Have your US Passport in hand, as you will be met by one or more heavily armed guards who speak very little English. Once they see your US Passport and that you are a US Citizen, they will ask you what your business is at the Embassy.
Vaccination requirements? VACCINATION REQUIREMENTS
(for immigrants only, not for K visa applicants)
Recent changes to US immigration law now require immigrant visa applicants to obtain certain vaccinations (listed below) prior to the issuance of an immigrant visa. Panel physicians who conduct medical examinations on behalf of immigrant visa applicants are now required to verify that immigrant visa applicants have met the new vaccination requirements, or that it is medically inappropriate for the visa applicant to receive one or more of the listed vaccinations:

mumps, tetanus, influenzae type B (HIB), influenza, measles, diphtheria, hepatitis B, rubella, toxoids, varicella, polio, pertussis, pneumococcal

The "Information Packet" (Combined Packets 3 & 4) contains information on the documentation you must provide at the time of the interview. The medical exams must be performed only by the physician identified in the packet. K3 and K4visa applicants are not required to submit proof of vaccinations or to undergo any vaccinations until they adjust status with the USCIS. Therefore applicants may wish to consider carrying their vaccination records with them to the United States to facilitate this process.

Visa applicants should consult with their regular health care provider to obtain a copy of their immunization record, if one is available. If you do not have a vaccination record, the panel physician will work with you to determine which vaccinations you may need to meet the requirement. Certain waivers of the vaccination requirement are available upon the recommendation of the panel physician.

Only a physician can determine which of the listed vaccinations are medically appropriate for you, given your age, medical history, and current medical condition.

Cheap and good hotels near the Consulate? Personal opinion... Stay on the right banks of the river in the city. Get a hotel near the Metro. I spend between $50-$100 on the hotels. You can stay at any hotel on the right banks. The consulate is near the red line Beresteiska station. The embassy is 15 minute walk from the station. Walk into the city and use the taxi services who solicit on the sides of the street. They usually are 50% less expensive than the hotel drivers. Alternatively, the apartments for rent on many of the Kiev Apartment websites offer driver services at reasonable rates. The Apartment Services range from $45.00 per night upward. Good Luck! New Embassy is outside of center, good connection with Metro from Kreschatik, Theater, Voksal. Metro and a little walk is quicker than a cab.
Additional Info? TRANSLATIONS: Documents in English, Ukrainian or Russian do not need to be translated. When you arrive for the interview, you MUST present all the original documents accompanied by an English translation, which is produced by a certified translator, only if the document is not in English, Ukrainian or Russian. After the interview, all originals except the medical examination report will be returned to you. The consular officer has the right to request translation of any submitted document.
PASSPORTS BY COURIER: The Consular Section uses a delivery services to return passports
All documents are valid 12 months?? from the date of issuance. Better check this one out. The US Embassy told us that the local police check report was valid only for 6 months after the date of issue.
Cost of the K1 visa is $265.00 to be paid in advance with a pre-printed deposit slip and deposited at a Raiffeisen Bank Aval branch. USD exchange rate at embassy determines the amount in hryvnia to be deposited at the bank (rate updated weekly).
