DS-156 - paper form not actually included. Instead you receive instructions on how to fill out the form electronically at http://evisaforms.state.gov/. Montreal will NOT accept the DS-156 if it is handwritten. Two copies of the DS-156 should be printed with the barcode generated by the online form.
Should you need to complete any handwritten forms, use the forms that Montreal sends you - they have been picky about this in the past. However, it has been confirmed that printing the I-134 from the USCIS website is perfectly fine.

Effective Jan 1/07, Montreal mails the visas out. You will need to take a 314mm x 394mm or 30.5cm x 39.4cm Xpresspost, pre-paid (Flat-rate), signature required envelope with you to the interview. Address it to yourself. Most have received the envelope back within 2 business days HOWEVER, be prepared to wait at least a week. Some VJ members have waited for 7+ business days! Best advice - write down the tracking number so you can check it via the Canada Post website. |

Days Inn downtown. A short cab ride/drive from the Embassy.
L’Auberge de Jeunesse de Montréal (Montreal Youth Hostel)
Anyway, it's a *really* short cab ride from the hostel to the consulate and is EXTREMELY cheap to stay there. I flew up and stayed one night. It has the basic necessities. Nothing fancy, but you just need a roof over your head and a shower/bathroom. Worked out great.
