I have several questions concerning my vaccination records from Cuba, I had my medical exam in Cuba and also my vaccinations records, I gave all this to the embassy the day of my interview and subsequent approval, when I came to US I gave the yellow envelope to the inmigration officers I suppose there was my medical exam, I don't have any copy of the medical exam. I was reading here at visajourney that USCIS gets my medical exam from the POE when I arrived here to USA then my question: why do I need to resubmit in the applicaction for AOS my vaccination records ? The inmigration officer have my medical exam and they didn't give me any copy so I don't have nothing to send in the AOS application regarding my vaccination records, what should I do if they are asking me to send my vaccination records ?

Any help would be really appreciate it !!!!
Thanks in advance,
Ariel

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.
