There is a thread dedicated to finding civil surgeons in certain areas, and people's experiences: http://www.visajourney.com/forums/topic/231026-civil-surgeons-who-dont-rip-you-off/page-26

Unfortunately you have to go through the whole thread to see if you can find what you are looking for, but it's a start. Are you adjusting status? I'm asking because you are asking about the immigration medical--something not applicable to student visas, and therefore you might not get a lot of answers in this specific forum.

I assume you are adjusting status? What visa are you adjusting from? The civil surgeon will complete form I-693 for you (you just fill out the Part 1 of the form), and you need to submit that with all the other forms (I-130, I-485, I-864, I-765 (optional), and I-131 (optional). All this is assuming that you are adjusting status (since you posted in the student visa forum) and are filing your AOS (I-485) and petition (I-130) concurrently.

It will be helpful if you can provide more information. I do not want to provide you with wrong information that is not applicable to your situation.

I paid directly after my first visit, but I don't know if it maybe varies depending on where you go. I suggest you call around to different civil surgeons and ask them how their payment works. Some also charge more than others.

You will send your medical results and form I-693 to USCIS with all your other forms. You will receive it in a sealed envelope from the civil surgeon. Don't open it, submit it as it is to USCIS.
