| All immigrant visa applicants must pass a medical examination before visas can be issued to them. The examination must be performed by a physician designated by the Embassy. The procedure consists of a physical examination (all applicants), chest x-ray (applicants 15 or older), HIV blood test (applicants 15 or older) and vaccines (if necessary). Do not bring the x-ray to your visa interview. Keep it as part of your permanent medical file. If you immigrate, pack the x-ray in your luggage. The medical examination fees (in Guyanese dollars) should not exceed the schedule outlined below. Note that this schedule does not include the cost of necessary vaccinations, which are subject to local pricing and availability. - Applicants age 14 and under - G$5,000.00 - Applicants ages 15 to 65 - G$15,000.00 - Applicants age 66 and over - G$11,500.00 Please be prepared to pay the physician’s fee in cash. If the doctor refers you for further tests or consultations, you will be responsible for any additional costs incurred. |

| Be prepared for the interview. Start collecting your documents early. I suggest the following: Financial Documents: 1. Tax Transcripts – Call the IRS for this. Its free and takes about 10 days to get it 2. Job Letter 3. Bank Letter 4. Bank Statements (at least 6 months) 5. Pay Stubs (at least 6 months) 6. Life Insurance Policy statement (if you have one) 7. 401K or retirement statement (if you have one) You MAY have all the above but the Embassy may only take ONE or 2 docs. In our case, the embassy only took my Tax Transcript for 2006 and my job letter…Nothing else. Its still a good idea to have the documents and not need them as opposed to needing the docs and NOT having them. Be sure to notarize your Affidavit of Support in the US. The Embassy charges $30 to notarize a document. Its $2 in Queens, NYC :=) Civil Documents (*** see note below ***): 1. Beneficiary Documents a. Birth Certificates b. Marriage Certificate c. Parents marriage and birth certificates i. If divorce, get a copy of the court statement 2. Petitioner a. Birth Certificates b. Marriage Certificate c. Parents marriage and birth certificates i. If divorce, get a copy of the court statement It is INSANELY IMPORTANT that you get UPDATED copies of your civil documents (also take the originals as well). Make sure your Birth Certificates and Marriage Certificates (parents, petitioner, Beneficiary) were issued within the LAST 6 MONTHS…NO EXCEPTION. If you listen to anyone who says your Birth Certificates and Marriage Certificates can be more than 1 year old then you will learn the hard way. Go to GPO and request copies. each document cost about $3,000 (GY) and you get 3 copies. *** I suggest you organize your documents into a binder and create an index page so it’s easy to find. Relationship evidence: Take EVERYTHING you have. Organize your cards, letters, etc into “HIS” and “HER” packets. Organize your photos, travel documents (boarding passes, reservations, etc). Other: a. It’s OK to be early. They will let you into the building BUT not the main waiting area. b. DO not attempt to take anything not permitted into the embassy. If they find something you are not allowed to take, for example, a wedding DVD/video, you will have to remove it and join the BACK of the line. If a DVD is required, try and get some kind of letter from the Consular stating that it is requested for the appointment. c. Be polite. Yes Sir/Mam. May I ask you a question. d. Check the website or Depart. of State site for the correct forms. These guys will sometimes send the wrong thing e. Include a cover letter whenever you are dropping off any document to the embassy. always include your case # and First, last Name |
