Doctor did not check waiver for flu shot on DS-3025

Hello Everyone,

My fiance just came here on a K-1 visa and we are looking into the AOS paperwork for after the wedding. He has a completed DS-3025, so I think that he doesn't need form I-693 and he doesn't need another medical exam. Everything appears to be good on the DS-3025 except for the flu shot. Almost all the vaccines either have a date by them or a letter in the waiver column. The only one that doesn't is the flu shot. There's no date listed because he didn't receive it. But even though his exam was in August (not during the October-March flu season) the doctor did not put the letter F for the "out of flu season" waiver. He just left the whole row blank. Is this ok? Do we need to go to a civil surgeon just to have them put an F in this box? By now it's flu season so he would probably have to get the shot if we go to a doctor. Or could we just write a note explaining that the date on the DS-3025 is outside of flu season so it was not necessary to receive the shot? In the summary section the doctor checked the box for "US vaccination requirements COMPLETE."

One other small question. I've heard that you can just send in a copy of the DS-3025, is that true? I would hate to receive an RFE over something as simple as not sending in the original, but I'm worried about sending them our original copy since we probably won't get it back.

Timeline

The original DS2035 from the medical was submitted in the sealed envelope that was taken from you at POE

and your own copy that the doctor gave you should be submitted with the AOS package. You make another copy for yourself to keep.

The original will meet up with you at AOS, they already have it. Sometimes they lose it, therefore you submit your own copy.

If everything is marked complete then don't worry. I would not get a transcription unless they RFE'ed.

UPDATE: July 5, 2015 updated on all 3 cases, RFE received June 30, 2015.

Service request : Aug 12, 2015, letter received that it will be processed within 90 days from receipt of RFE.

UPDATE: Aug 24, 2015, EAD card being produced/ordered. ( 102 days from AOS receipt day and 55 days from RFE response received.) Thank you Jesus !

Emails : Aug 24, 2015, EAD approved, EAD card ordered.

Sept. 21 Interview, 2 hour interview, we were separated and asked about 50 questions each for an hour each. IO was firm but professional, some smiles.
Several service requests made, contacted Senator and Ombudsman. Background checks still pending.
July 21, 2017 HOME VISIT.  Went well. Topic thread in AOS forum.
Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

Agree - I was missing an immunization date that wasn't transcribed on the DS-3025 and I had to call 5+ civil surgeons in the area to finally find one who would agree to do the correct vaccination transcribing now on the I-693 for $120 (!!! - it only cost me $15 in Canada). All the other CS's were insisting I have another full physical which cost $250+. It's like they hear immigration and think $$$$. I told them if they read the form instructions they would know I had one already and it wasn't required for my K1 visa and they simply refused to do it. I couldn't believe it - it's a money scam.

And the CS also required I get a flu shot (because it's now 2017 flu season)... and I had the 2016 flu shot with my physical prior to entering the US.

In short, it was a hassle and I would wait for the RFE - they should look at the August date and put 2 and 2 together.

I wouldn't hesitate to write a note if it puts your mind at ease. ;) Make sure it's attached to the referenced form/page.

Thank you both for you help and discussion, guys! One more thing, I just want to make sure I understand this correctly. Since the ds2035 is complete (sort of lol) I don't have to submit the I-693 at all, right? Assuming that my fiance's vaccines are sufficient, I just attach the DS 2035 to my I-485 and leave out the I-693 completely, right?

In case anyone else in the same situation reads this post looking for advice

 

I submitted the DS2035 vaccination sheet without any note or explanation and we just got our green card approval a couple weeks ago! No RFEs, no problems. So it looks like USCIS could figure out that we didn't need the flue vaccine, even though the doctor didn't check the box.
