Long form birth certificate

Hi All,

I sent my step children's mother to go get copies of their birth certificate in this "long form" that I keep reading about. She said she went and they looked at her crazy and said there is no "long form" birth certificate.

What is up with that? How does she obtain it if they tell her it doesn't exist. If anyone has gotten their long form birth certificate, can you tell me where you went and what exactly makes it different from the birth certificate we already have.

Where are they born? If foreign country maybe they call it something else. USCIS is basically looking for on a birth certificate: DOB, Place of birth, parents name and hopefully their date and place of birth (nationality)the long form birth certificates are usually required for Americans because we have the option of short/long form. So if you are the American citizen then your BC needs to be long form.

They were born in Nigeria and many people from the nigeria portal mention they need a long form.

I'm just confused.

You do not Need the Long Birth Certificate

Especially if the birth is in 90s upward.

If she has the Birth Certificate that was issued at the Hospital

That should do it .

It Just must be from National Population Commission of Nigeria

According to the State Department website the Long Form Birth Certificate is a requirement...

Birth Certificates

You (and any family members immigrating with you to the United States) must obtain an original birth certificate issued by the official custodian of birth records in your country of birth, showing your date of birth, place of birth, and parentage

Important Notice: All Immigrant Visa applicants must submit a long form original birth certificate. Short form birth certificates will not be accepted.

The certificate must contain the:

• Names of both parents

This question has come up so many times that I have now written the consulate in order to find where to obtain this LONG FORM birth certificate. I will let you all know as soon as I have this information.

For Nigerians, it's two forms. Go to the National Population Commission of Nigeria in Lagos. They'll tell you how to do it. An overview is that the mother of the child will give two passport photos and signs an affidavit of the child's birth. They also need a passport photo of the child. Then the National Population Commission will give you a birth attestation. The US consultant needs both of these forms. The NPC in Sululere. You don't ask for a "long form birth certificate." Tell them you need a birth certificate for the US consulate. It shouldn't be more than 5500N.

I will ask my husband when he gets home from work as he had to obtain his birth certificate. By the way at the top of his certificate it has the heading of "Lagos Mainland Local Government, Health, and Environmental Services Dpartment", I can't even begin to tell you where that is

Also once she gets to the national population in Lagos she should tell them she need birth certificates that the embassy will accept for international travel and they will issue her the correct one......
