Confusion on form to fill out for Visa exemption

OK, I am trying to figure this out, but as some of you know I am dyslexic and with such small lettering all together I sometimes just get lost trying to get everything straight.

This is a capture of the document (below) I don't know what I am supposed to do for parts 11 and 12. My wife is the Vietnamese citizen, so I am applying for a five year visa for ME. Not her. So what am I checking on part 11?

I know I am supposed to send them MY passport, a copy of our marriage certificate, copies of my wife's Vietnamese passport pages.

The form to fill out is confusing.

Item 11: check mark "Certificate of Marriage" and send in a copy of your certificate.

Item 12: If you're going to send in your passport, check mark "A stamped certificate" and they'll put a stamp/sticker on one of your pages.

I sent mine in almost 2 weeks ago. Last Friday would have been the 7th business day that they had it. the website claims that they will process in 7 working days. I expect it back any day now and will advise if what i did worked. I do know that they cashed my check last week so i take that as a good sign.

If it's the 1st stamp in your book, it'll be $20. If you're getting a renewal it'll be $10.

New passport book=1st stamp=$20.

As I stated, being dyslexic, I have a terrible time with documents written in two languages and this one is a nightmare. My eyes cannot focus and I just can't read it. I've been working on this for two weeks. Two ****ing weeks now.

So at the moment I still cannot find the address to send this to.

I cannot find who to write the check out to.

And I am not sure if I need to include a postage paid envelope for them to return my passport.

When I call them, I cannot get an answer to the phone, or they don't speak English.

Honestly, I am ready to pull my hair out.

No worries...I just didn't know if you were trying to fill it out in Vietnamese (but then I saw that you actually said that the error message was in Vietnamese...).

I sent it to the Embassy of Vietnam in Washington, DC. I made the $20 check out to The Embassy of Vietnam. they cashed it so I guess that part worked. Now just waiting for it to come back.

FYI, for anyone planning to do this soon:

I just spoke with the Embassy of Vietnam in Washington, DC. The lady told me that they are getting a lot of requests now for visa exemption and they are behind. It is now taking 15 working days to process.

Also, she said to be sure to include a trackable, pre-addressed envelope with postage paid so they can send it back to you. Better information is available here for those who can read VN:

Send mine to DC this morning, with a Priority Mail postage paid (including Certified) inside the box. Hopefully I sent them what they needed.

My passport

Application

Copy of our marriage license/cert

Color copies of my wife's Vietnamese passport

Two Passport type photos of myself

$20 check

Based upon what I am reading here. I can expect to receive my passport with stamp back around December 5 - 10. About five weeks before we leave. God help me if they don't give me the visa and I have to start over again.
