Overall, I had the impression that the Tokyo Immigration Unit was pretty efficient. It took only 5 days from them receiving the case from NVC to sending me an instruction email. Thankfully I had recommendations from a community member who went through the process a month before me, so I could do all the necessary things (book medical, schedule interview on online system etc) before even receiving the instruction email. Have to say, the instruction email was not very informative besides asking for documents.
For the interview itself, I did not wait too long until an officer checked my documents. As a Japanese citizen born outside of the country, they were very weird about not wanting to accept my original birth certificate from my country of birth. They needed a Japanese certificate of birth, which resulted in my first extra document that was requested (mind you, this was never mentioned on their website as necessary for Japanese nationals). My fiancee had nothing but positive things to say about the interview experience. Everyone, including security, reception, and the interviewer was very nice and helpful. She stayed at a hotel 5 minutes from the embassy and got there early. The whole process was finished in about an hour.
We went over the I-129 and I-134 info several times beforehand and did some practice interviews together which she said helped a lot.

During the interview, some documents were missing, and I was asked to provide additional paperwork. As a result, it took longer to receive approval.

~2 hour wait after document submission followed by interview of less than 5 minutes. Two or three questions of 'how did you meet your spouse', 'what does he do', and done!

mostly foreigners there for immigration visas
the lobby was loud
couldn't hear what the interviewer was saying
