I can tell you about 100 people who didn't because they got it complete at their visa medical. A photocopy of your DS-3025 with Tdap/Td/DTP etc, MMR, and VH (by varicella)on it; the "results" marked incomplete-may be eligible for blanket waiver; and the doctor's signature will be considered acceptable.
