Shpat, I have a severe accident in 2006, and I have a disability, I am a physical handicap, I need help in the most of my daily routine and medication for the pain.

I have post-traumatic stress disorder, anxiety, flash backs, depression, ... and bunches of problems in my everyday life!!!

The good news is that I am a miracle, God is doing a wonderful work with me! I am alive and now I can walk again!

Unbelievable but True, The only thing that they need is a new form!!! I am sure, they lost our original form!!!! GRRRRRRR!!!!!
