  1. In San Diego I ended up finding a place that filled out the form without requiring another full medical exam, they needed the previous vaccine info filled out by the doctor in Kenya (which we had) and proof of the new vaccine which we had evidence of from a CVS print out that we requested when getting the vaccine. The health clinic charged us $350 but the doctor knew exactly what to do and finished it in 15min.   2. Called Del Mar Integrative Medicine and Roger Kasendorf, both listed on USCIS, rebuked by both saying they would only fill in the full medical. I'm probably going to call 2-3 more, but might just go ahead and do the full medical to move past this, will get the Polio vaccine at CVS or somewhere first, and we will bring the worksheet of vaccine's already completed in overseas medical appointment. Thanks to those who responded, we appreciate it.
  3. Just had our Green Card interview, apparently Polio vaccine is now required (wasn't done on wife's overseas medical exam and vaccines), it's the only vaccine/item missing. Case officer gave us an RFE, and said she does not need to redo the full medical exam, just select checkbox in Part 1, 4A on I-693 so you can skip the medical exam part of the form, and they can use her existing vaccine record info from home countries' vaccination documentation worksheet done by panel physician as proof and to fill out that section for those vaccines in the I-693. I looked up civil surgeon, first one I called said they only do the full medical exam, they "wouldn't feel comfortable signing off on another physicans exam", to which I said that part of the form is clearly left blank/empty based on the checkbox we select in Part 1, 4A. They said to try calling other places. Anyone have any advice in this situation? Even if we go get the polio vaccine at like a CVS or somewhere, and take it to a civil surgeon, will they use her US Dept of State vaccine documentation worksheet from her home country to fill in the vaccine section? Will we be able to find anyone that will agree in skipping the entire medical exam portion that was already completed and that our case officer and the form itself has said is not needed?
