  1. I just checked my DS-3025 and I had a couple of questions. My Hep B vaccine on it has a B waiver marking insufficient time interval to complete the series. Wouldn't that mean I can just send the DS-3025 in instead of the i693? The doctor did not mark "Immigrant Visa or Parolee applicant completed vaccination requirements" but "K Visa applicant electing not to be vaccinated at this examination". Need to specify I was never asked anything as they don't offer vaccines there and the only thing I was told was I needed the last 2 doses of the Hep B. I have DTap, TT (Tetanus), IPV (polio), MMR, Varicella history, Influenza and Covid marked off on it. Am I missing anything else?

The civil surgeon is okay with filing and signing the I-693, he just refused to use my DS-3025 as proof and won't mark my MMR and my Covid (even though I told him the Covid shot got waived by the latest requirements). He will be adding my DS-3025 and my vaccine paperwork from my home country as extra documentation on my i693, he just would not accept the DS-3025 as proof and won't sign those 2 off.
  3. The only vaccine papers I have are from my family doctor documenting everything I've had since I was a child. The system is very out of date compared to most Western countries and everything is on paper. These go back to the 90s so I'm not sure if they have the serial numbers/batch numbers in their official ledger but for my record they only put the dates in. They mark everything needed but the doctor's sheet has no section for MMR, only Mumps so my MMR was marked there.
  4. I called my overseas physician to get a list of the required vaccines when I made my appointment but I could not get the last 2 doses of the Hep B since they had to be spaced out. That's not an error. I had everything else completed. They should have marked me off as completed since there was not enough time for the last 2 doses.
  5. I showed my original vaccine records from my home country along with a translation, there was only a small issue. On my family doctor's vaccine chart (Eastern European country so not the most up to date on paperwork and stuff like that) there is no place to mark Mumps, there is only a Measles rubric. So my MMR was just marked there and the civil surgeon refused to acknowledge that I have the MMR even though it is marked on the DS-3025. I tried to explain but they did not believe me.

  7. My DS-3025 is marked off as K Visa applicant electing not to be vaccinated at this examination. I was not asked if I wanted to get vaccinated as the place did not do vaccinations and it was the only one in my country. The doctor simply told me I needed 2 more doses of the Hep B and to bring those along with my DS-3025 to the civil surgeon. Unfortunately In my location there are only 2 Civil Surgeons, the one I went to and another one that charges an extra $300 for blood work so this was my only choice. I checked the report on my visit on the clinic online portal and they marked me off as not having the MMR and the Covid even though my DS-3025 clearly has them marked off. I feel like they screwed up from the beginning of my medical as the first thing the nurse told me when I went after they checked all the documents I provided is was that I did not have any vaccinations at all and refused to let me explain or ask anything. I did get fully vaccinated overseas. I had everything but the last 2 doses of the Hep B and the doctor told me to get them and bring them to the civil surgeon. I have every single thing marked off at the time of my medical except the 2 Hep B doses but my DS-3025 is not marked off as complete.
  8. Hello, I am very distraught following my medical in the US for my i693. I had my medical yesterday and as a K1 visa holder I have previously had a medical in my home country and was given a DS-3025 because I was missing my last 2 doses of Hep B so I was told to get these and bring them with the DS-3025 to the civil surgeon for them to mark off my vaccines. However during my medical I was told they will not accept the DS-3025 as proof of vaccinations because it does not have lot numbers on it. I tried to explain it is an official document and I was told anyone can fake it and it is not guaranteed the US Government will accept it. I know I am in the right here, is there anything I can do? They had not even heard of the DS-3025.
  9. I tried looking this up but I could not find anything relating to these specific circumstances. K1 finally got approved, moved to the US, married in Las Vegas the next month. Have not applied for AOS yet. Here lies the problem. For personal reasons I want to change my last name to my grandmother's last name and my wife would be taking it as well and it would be our (married) family name. I could not find any relevant information about what to do in this situation. Some people have said to just apply for AOS under the new name, some people say your chosen name has to appear somewhere on the marriage certificate. What do I do in this situation? I would really appreciate any kind of help. If possible, I do not want to AOS under my current last name.
