i asked this question again because no one answered it, only a few questions were answered, not all of them.
And yes, this is for AOS, but i figured i would state that we have the K-1 Visa, i wasn't sure if the K-1 process was different from any other visa when filing for the AOS.
btw, what does FWIW mean? im not familiar with all the codes yet..

Does she have the vaccination documentation worksheet from her homecountry?
If so this can be put on the I693 form from a panel physican here.
Dr. L E Arnold in Texas does it for 10 Dollars.
You can mail him everything so you dont have to go there in Person.
If you have the DS 3025 then you have what you need... if not you need her vaccination records from her and get them transcribed by a civil surgeon...

Dr Arnold is a woman and she now wants $20
They charged me $388.00
for varicella,TD shots, Hepatitis B & two forms of I-693. did you think they charged too much?
no that sounds about right...in GA, the cheapest i found was $295, but thats from the ones i have seen, there might be someone cheaper but idk?
They are also saying someone in TX only charges $10...that doesn't make sense to me because i dont know of any doctor to charge $10 for even one shot, i went to get a flu shot at our local health department, and they charged my $20 alone for that so they might be referring to something else, not to sure.
But we needed a civil surgeon to do the complete medical process in order to send the I-693...so in my opinion, $388 sounds about right.

i was told that you will need 2 passport like pictures for the following forms:
I-485
I-765
I-131
so i am placing 2 photos behind each form, followed by his passport information, ect...
if i am wrong, someone please correct me!!
Do remember to write his name on the back of each photo.

How should i put all the papers for my AOS packet in the sealed envelope when i send it off? Should i put it in a folder, use any kind of clip, such as ACCO clips?
Or should i keep the papers loosely and just place them in the envelope like that?
And for the $1010, hat do i attach the check to? should it be in a separate envelope or anything?

The civil surgeon we went to was wonderful!! and he charged $295 for everything, which is cheap compared to everyone else i called....just look up civil surgeons in your area on the uscis website and then just go down the list and call for prices...thats what i did.

Well the doctor in Texas called L E Arnold writes the DS form into the I693 for 10 bucks.
Or did you need the full medical exam??
yah, we needed the full exam...the other civil surgeons were charging between $400-$550 around here in georgia. So the one we went to only charged $295...it was cheaper than the others, so we went with him.
I also think that its expensive, its kind of crazy for the amount that they all charge, we were in the waiting room for 30 mins
and once we went into the room to get his shots, it took less than 10 mins! so i think its insane that they charge that much!! it shouldn't be no more than $100, in my opinion!

We went to walmart and got my husbands passport pictures done yesterday, we only got 2 but we need a total of 6 photos for his packet.
Would it be okay if we just scanned the photo onto my computer and then printed it out on photo paper? or will this not be accepted?

I just want to know, where should i put the check, should i put it in an envelope? How did you all do this?
How did everyone organize their packet, did you use an ACCO clip? folder? or did you just stick all of the papers loosely in the big envelope?

The last three years don't matter unless you are self employed. Only the most recent 1040 filing matters.
Whether or not your father's income is sufficent depends on how many people are in his household.
4 people are in our household...myself, my daughter, my husband and my father

Okay, im so confused!! let me just tell you what i have for our AOS packet already filled out so far:
I-485 - filled out and signed
I-765 - filled out and signed
1-131 - filled out and signed
G-325A - all 5 pages filled out and signed
now those, i understood and had no problems...now with the Affidavit Support forms, im confused!
Which forms do i need to fill out for our Affidavit Support?
My husband and i have a daughter together and we live with my father, so my father will be the joint sponsor, since i didnt make enough for the 125% poverty thing.
So i filled out the I-864 for myself as the sponsor for my husband
and then my father filled out the I-864A to be the joint sponsor.
Are these the correct forms? Do i need to fill out more? I saw the I-134, do i need to fill that one out as well?
please help!!!!
Edit: We are doing the K-1 Visa, if you need to know that information.

I seen that the sponsors have to have a certain amount that they have to make or whatever its called, im not sure...but i hope you all know what i am talking about. Here is what i made within the last 3 years (and its not much AT ALL!)
2005- $5,663
2006- $5,845
2007- $1,130
Here is my fathers last 3 years of income:
2005- $28,050
2006- $21,324
2007- $26,550
My father just got a big promotion and he is bringing in a lot more salary this year 2008...do i need to get him to give me a copy of any of his recent pay stubs?
Those are the amounts that my father and I both have for the last 3 years...will they accept this or is it not enough?

Thanks for leaving your email in that blog, I really would appreciate if you helped me out a bit.
I've been dating my girlfriend for 6 months now, she came from the Phillipines on a tourist visa (6 months) then got a 6 month extension, before that expired she was sponsored by an agency who places registered nurses in to good jobs, so she started working in NYC about 6 months ago on a working visa. She has files all her paperwork and has been accepted for a greencard but tells me that due to some cheaters, she needs to go back to the phillipines to retake an exam, she would be gone for 2-3 months to study and retake the exam, then after passing she would have completed and complied with everything for her Visa Screen.
This was not a problem, I assumed she could go take the test and come back since all her paper work was in order. A few days ago she gets a letter from her agencty saying she has until the end of Feb to comply and file for her visa screen. That is impossible due to the fact that the next available retest date is sometime in June.
I asked her why she did not take care of this sooner and she said she had no idea that she would be filed so early and thus accepted back in Jan of 07, which I guess gave her 1 year til then to have a visa screen. There was a test back in the phillipnies bak in Dec and I asked why she didnt go back and she said she had no money which is a pretty lame excuse for something So important.
The fact is that since this whole thing came up I offered to just marry her on paper and test the waters in our relationship for a couple years while we wait for her citizenship and if its meant to be we would get married the right way,. for real. She asked me many times if I was sure and I said Yes, I would do it, Only Beacuse she told me everything was in order and she would be getting her greencard eventually, 3-4 years later. From the time we started dating I told her I would never be that guy to marry her for a greencard because I wanted to know that her love for me was real.
She has professed her love and devotion for me since the second week we dated, Its a long distance relationship by the way, her in NY and me in Chicago, I met her on a biz trip and have flown back 3-4 times and she has flown here 3 times, she even wanted to spend Christmas with me and my family so she came over and I was quite uncomfortable about it but it turned out ok. It just seemed way too soon to start doing family holidays together. I'm Greek, she is Chinese, the distance and the way I met her has always affected my trust issues. I met her online on a free dating site, she also had a myspace account, her profile was filled with very pretty, sexy / seductive poses and pics. so I still have trust issues, she crys and tells me she never lied to me.
I have fallen in Love with this girl and I'm scared I fell in love under false pretenses or will end up being humiliated and hurt if she had this planned all along. Since I went back on my word today and told her I can't go thru w the marriage thing she has been sobbing and scared. Too many people have told me to NOT do something so stupid if I'm not 100% sure about our relationship, my sister believes she is in Love w me and I do to. But what risk do I take by doing this, if I feel she is the one for me, but had me fooled from day one. I went to my bank to get a statement saying that we share a checking account, the lady printed out a page stating that we have a checking account together and it has my information and everything on it but it doesn't have the Wachovia letter head on top, will this be acceptable? or do I need to go back and get another one with a letter head?

Or do we send a COPY of the certified copy of our marriage license?

If you are going to file for the I-765 and the I-131 as well, you will also need 2 passport photo like pictures for those forms, so you will need a total of 6 passport like photos for the whole packet..what we did was, we went to walmart and got one set of passport pictures for $10 then we scanned them on my computer and printed 4 more on photo paper, people on here said that it shouldn't be a problem, so i hope that its not!
Make sure that if you are the sponsor that you meet the 125% poverty line for everyone in your household, if not, you will need a co-sponsor or a joint sponsor (if someone else is working and is living with you) This is what i went by:

The G-325A - it will be 4 forms, all of them are the same and they need to be signed and dated and sent in, dont only send in one, send ALL 4!
If you have already had a medical exam then you wont need to see a civil surgeon, we had to go to one to get the I-693...we found a guy that charged us $295 for everything, he was the cheapest in GA, from what we found, most charged over $400, some even $500! its crazy! But if you have to see a civil surgeon as well, go to uscis.gov and look for civil surgeons that are in your area...be sure to call around your state to make sure you get a good surgeon, there are a few here in GA who rip you off and dont do everything that you need done!
I am sending out my AOS packet tomorrow, i am just telling you all the things that i didnt know about at first and thought i would share what i have already learned so far. I hope i helped you out some!
