DS-5540 is currently not required. Before your spouse's interview, check back here or the Department of State's website to see if that changes -- https://travel.state.gov/content/travel/en/News/visas-news/update-on-public-charge.html

Do not do that. Wait until NVC has scheduled your interview first. The visa will probably expire 6 months after the medical (if not earlier).
