Presuming that you are not a graduate of a medical school in the US or in Canada, you just need to answer whether you have not yet passed the national board of medical examiners (eg, all three steps of USMLE) or its equivalent.
If you have not passed the national board of medical examiners or its equivalent, your answer is yes.
If you have passed the national board of medical examiners or its equivalent, your answer is no.
It doesn’t matter for your visa what you answer because DV doesn’t require labor certification. The correct answer is yes and at interview if they ask just say you’ll get the certification once in the US before you practice.
