I do not know how the Embassy handles that in Pakistan but the psych exam in the Philippines has to be done by the Embassy approved medical facility and only after the exam is ordered by that medical facilities doctors.  I assume you can bring paperwork with you but the Embassy probably requires the exam be done by their doctor and in accordance to the requirements of the US Surgeon General.

I had my medical for the K1 in Germany. My old scars were no issue. But i brought a signed statement from my doctor confirming that I was healed and was no longer posing a threat to myself (or others).
