They don't really need anybody's life story, so yes, try to be short.
"... when she was in the US, we were dating, but then after she left we ended the relationship. Few years after, we reconnected again, and I flew to her home country several times where eventually I proposed.." - just like that nice, short and clear explanation how you two met and started your relationship.
About documents - send everything you have that can proof you are in ongoing relationship. So if you have something from that "past", I would suggest to attach it as well.

1) yes....you need to mention it on your application in the proper section
2) no. a photocopy of the signature is fine
3) yes. that counts as having met unless it was more than 2 years before filing for i-129F. ongoing relationship is evidence with chat, videologs and other social media, email and more. ongoing is clear with snail mail, email etc with proper dates on it. key is having communicated periodically after meeting
4) intent to marry needs to be from each one. separate. clearly 2 different intent to marry that are signed. no originals needed
