Yes almost everyone gets an AP and each AP will take its own time.  Apparently AP for parents/siblings is taking the longest, then comes AP for fiance and then AP for a spouse. Males are more likely to get an AP because of a common name. AP for a missing document takes the shortest. However, these are not always true; each AP is a case of its own and will take its own time, it cannot be compared to other APs. The quantity and type of red flags in the case determine how long it should take and unfortunately there is no straightforward method to find out the red flag.

What would the online status change to after 'issued'?
 
right now the statement reads "Your visa has been printed. Depending on local procedures at the location where you were interviewed, your visa will be mailed or available for pickup soon. "

case for wife and son was in AP and last updated was 3-Feb. Today it is showing issued. Does this mean that it's sent to their address?

I just tried it and had it work. I think it may be fixed.

Yep, it's currently broke. From what I read, their scheduled maintenance window was Saturday but they must have had some issues. It's Sunday as I type this and it's behaving the same way for me.

No, I was working in Pakistan and my yearly salary was below the filing limit. So when I had to send the NVC bundle I sent a letter explaining the situation that during 2016-15 I was working abroad and my income was below the filing limit and hence the tax return was unavailable and that I moved to the US at XX date and attached a proof of employment letter from my HR along with recent paystubs.
