Reply with "YES" and list the name and address of the agency. Don't play with USCIS! You paid money, you used a 3rd party agency. There's no GRAY area there!

Did you sign it or not? Sounds like you're unsure! If you didn't, you will receive an RFE. Don't sweat it.

You don't need a Wedding announcement! In fact, the USCIS advises that you do not make plans until after you've received the Visa. You don't need it!

If you paid money for a name and an address, you answer YES and give the name and address of the agency that you paid money to.

I and my Brazilian Fiancee want to complete what we can in advance. We can make sure we have perfectly correct information and we won't need to send information back and forth under pressure!
As I understand things, the American Consulate in Brazil will send her a Packet#3 and Packet#4. Right now, I know that these are some of the forms that may need to be completed:
DS-156
DS-156K
DS-157
DS-230 (Part I)
FS-257A
DS-2001
..and the Police reports
But, are all of these required or can we ignore 1 or more of these? She stresses at everything and I want to reduce that and to make certain that the process continues without any delays.

You can make the I-129F application and prove your US citizenship with a Birth Certificate, Naturalization Document or by submitting a notarized copy of all the pages in your US Passport. But for $165, I'd go ahead and get the document. It's always good to have the original documents in hand, should you need them at some point in the future.
