    I waitied several hours from when I first walked into the clinic until I was seen but the Civil Surgeon did transcribe all my vaccinations (I had to have additional ones in the UK after the original medical) and it was $28. They gave me a copy of the i693 and sealed a copy in an envelope with their official stamp etc.

    Ok, my husband's ds-3025 sheet is not complete as he is missing the Tdap and also the date for MMR is not written. We are going to a civil surgeon, but now (March 13) is still considered flu season. The thing is I am terrified of flu shots, I am afraid he's going to get sick and also that I will get sick and I am pregnant. It's a phobia from the vaccine thinking that each time you take it you actually get it, and everyone around me has had that reaction.

    My question is, should I go ahead and subscribe it to the I-693 without the flu shot thinking that when they actually get to the case the flu season will be done?

    or wait for 17 days and then do it on April 1st?

  3. Hi Everyone,

    I want to help those who are getting their medical exam done soon and are of course going to transition from a k1 visa to a permanent residence.

    My husband had his medical done in Riyadh, Saudi Arabia. The doctor charged him $260 for 2 vaccines that he absolutely didn't need. And the ones he needed she didn't give to him.

    I don't want you to go through the same mess, so here are the vaccines you ought to have in order to do your AOS (Adjustment of status) when you arrive in the US

    Make sure have a signed copy of the DS-3025 sheet (part of the medical exam form)

    You have to have :

    1. Shots marked with a date (at least one of a series)

    MMR (two are required but if you have "insufficient time interval" checked, then you get a waiver)
    Td or Tdap or DT or DTP or DtaP (One no longer than 10 years ago, ie.have a booster)
    Varicella or if you have had chickenpox before the doctor needs to write VH (Varicella History) so you don't have to take it
    Influenza (during flu season which is October 1st to March 31)

    2. Everything else marked not age appropriate, insufficient time interval, not flu season...things that allow a waiver. Or if you have taken other just make sure you write the date, but they are not required for AOS

    It will save you time and money, because if the DS-3025 is not filled properly you have to go to a civil surgeon in the US and have them certify another vaccination sheet (I-693) which could cost you a lot.

    Hope it helps, and ps you don't need another medical exam once you arrive to the US if your medical was done within a year from the date you apply for your AOS

  4. Hi everyone,

    I have been having a lot of trouble finding a civil surgeon in the Plano/Allen or Dallas area that would just transcribe the I-693 vaccination sheet without requiring the whole physical. My husband did his physical within a couple of months (k1 visa) overseas but his vaccination sheet (ds-3025) is incomplete and needs to be transcribed by a civil surgeon.

    Please let me know if you know any in this area

  5. thanks, no my husband is only 30 years old. I am guessing the Dr really sucked, even though she is a civil surgeon, she gave him the Hib vaccine which is not required for healthy adults.

    It's just a mess

    I do have another question though, during the time of the exam the flu season didn't start, but now it did, should I have him see a civil surgeon to get it and have it transcribed into the form or it's not necessary?

    My husband did his medical exam overseas, and the Dr never gave him a copy of his DS-3025, but we contacted her afterwards and she basically rewrote it.

    She didn't list any dates on the previous vaccines he's taken, is that going to be a problem?

    Also wrote 'Not age appropriate' next to MMR, is that right?

    Please look at the attached sheet and let me know what should be done.

    we already received our k1 visa and got married. My husband had his medical exam done in Saudi Arabia few 3 months ago, with the vaccines of course. We have a copy of the vaccines he received But how do we make sure that they're enough?

    I looked at form I-693, it does lists all the vaccines. But it says that the surgeon may decide to waive some according to age and gender etc.

    So where do we go from here?

    My fiance received his k1 visa a couple of months back. In all of the applications and paper work that we filled , his name was as the following for example:

    First: A

    Middle: B

    Last: C

    But his passport has an additional name between the middle and last as all arabic names have that: E

    But we were surprised when the social security office worker told us that the visa joined the first and middle name together and made them as the first name : A B

    the middle as : E

    and last as C

    What can I do to correct his and who do I contact to fix this?

    your dad makes more than enough, so here is what you need:

    I know you don't have enough income but you still need to provide the following:

    your own I-134 form, letter from employer, bank statements, and 3 yrs of previous tax returns (if you don't have some of these it's ok)

    So your dad can be the sole co-sponsor, and he needs everything I mentioned above.

    If your parents have a joint account that's ok, and if they file their taxes jointly your dad can be a sole co-sponsor or you can fill both their names in one I-134 form. But you can't have 2 forms to my knowledge because that would mean 2 co-sponsors

    what I like to do is be prepared, so prepare a I-134 with just your dad and his supporting documents (mentioned above). And give this to them. If they're happy then yeeey, if not make an I-134 form with both your parents information and take it with you. Don't give it to them unless they won't accept your dad's (which they should but you never know) and they ask that your mom be included because the bank account is under both of their names.

    Remember, just give them what they ask for, and be prepared.

    Yes it was put to AP

    From date missing paper was received by the embassy to date the visa was issued is 7 Days

  11. I am soooooooooooo Happy we got our Visa today. Our interview was October 21st, my fiance was missing a paper and sent it 2 weeks later. We were in AP from November 11 to the 18th. Yes only 7 days, then it was issued then we got the passport today. YEEEEY I want to thank Visa Journey and everyone who helped me, this is the best forum EVER.

  12. Thanks @ gedaa86! My fiancé is going to have his interview sometime in November at the consulate in Costa Rica. I have been living in CR for three years and haven't filed taxes because I make under the minimum to file for US citizens living abroad... That being said I will only be able to turn in W2s/Federal taxes from 2010. The bank and employee letter are not a problem, but I don't know what to do???

    I think the best thing is giver your fiance what you have, and since you are missing the past 2 yrs of tax returns, have the co-sponsor give you all (employer letter, pay stubs, tax returns( 3 yrs is enough) and bank statements)

    And remember when your fiance goes to the interview, if they are not satisfied they will give you a chance to bring what is missing. So don't worry and hopefully everything will be ok :) Good luck

  13. Well, I think you really have to ask the embassy or a lawyer. You definitely need the employer letter. And I don't know about tax returns but definitely bring the 2010 and 2009 if you have them. Please ask a professional and if you can't find an answer, that's ok let your fiance go to the interview with what you have and if the consulate is not satisfied they will tell you what you need.
