Follow the-instructions .. if it says “ leave blank” … and it applies to you .. then do so. Elsewhere use N/A or NONE.
Follow the form and instructions. Despite our thoughts , they aren't intentionally planning to confuse you. Birth certificate shows familial relationship, US passport or naturalization certificate shows citizenship

I know, it's rough :/. We submitted a lease that had me as the tenant and my husband as the authorized occupant. They seemed to have missed his name though...
I submitted a bank statement from our joint bank account that has both our names on it. We also submitted plane ticket boarding passes which they didn't like because boarding passes don't have years on them. We submitted our ID's with a same address, some pictures, some post cards that were sent to both of us. Also the 2015 tax return which shows that we filed jointly as a married couple.
