Hi,

On the embassy packet 3 form DS-156, question 2 asks for the CITY and STATE/PROVENCE of issuance in regards to your passport. I do not see this information on the UK passport, under AUTHORITY it just says UKPA...

I have searched the forum and looked at the guide but I could not find a satisfactoy answer.

Any advise as to where to find the answer...or the answer itself would be much appreciated.

Many thanks!

Thank you, Gillian, but why London? Is that what everone puts in or was that because you were in London when your visa was delivered to you? Is the issuance something to do with your address where your passport was issued to you or the passport office that it came out of? I am probably making a bigger deal out of this then it is but..these forms do my head in...and the VJ guide just helped me confuse myself further because all the answers are (or all roads lead to) ROME. I know the passport office that we dealt with was in Wales but when we contacted the passport office last night they said the office was in Peterbourough Cambridgeshire (sp?)

Aggghhh....Confused!

The correct spelling is Peterborough, Cambridgeshire (where my sweetie is from!).

You could just put London down, as that is where the main government is located.

It would be like listing Washington D.C. for a USC passport.

Hi,

I just filled my DS156 in and returned it. Our lawyers, who have been great, did most of ours and they just put "UK passprt agency" in for place off issuance etc and left the rest blank, and i submitted it fine. I think if it is a problem the embassy website does not let you print it out anyway, and it accepted that. I do not think its really a big deal, the main thing they want to know is the country who issued it, and they can check it on the day of the interview, or against the passport number you give them.

Good luck....let me know when you get your interview...i am really curious as I am waiting to hear about my interview date now!

Thanks everyone, ANY and All advise is, as ever, so much appreciated!

All these forms...I can't imagine having had to go thru this process without VJ.

I know I am probably overthinking it...just so desperate for it all to be correct.

Cheers

Griff took the approach you mentioned above, as in Peterborough / Cambridgeshire and had no problems. So either that or London seems to work well as both Griff and English Muffin have had successful interviews in London!
