Passport questions on form DS 156

Adam is filling out the DS 156, and it asks for the city, province, and country that issued the passport. All it says on his passport is "country issuing code" GBR (great britain, I assume).

What does everyone suggest we put for the city and province questions? N/A? GBR for each?
