Timeline

The is what they sent us when we called for my husband's vac. record. There is no name of who administered the shot or anything 'official.' I think somoene else on here said this is the same thing they got and it was ok but I just want to make sure.

Will this pass for his vac. record? Will they ask to see more? We have another card from a more recent tetanus shot. But other than that, this is all we have.

8-14-2007 Mailed in husband's I-130 to Consulate in Toronto

8-15-2007 Toronto received I-130

8-27-2007 Toronto called to set up I-130 appointment

8-31-2007 Interview at Consulate Approved

9-25-2007 Received Packet 3 in mail

12-9-2007 Received police record (fingerprint version)

1-18-2008 Sent packet 3 back

2-26-2008 heard back from Montreal via email about our interview date

4-23-2008 Montreal Interview!!! Visa APPROVED!!

5-31-2008 Crossed the border into the US to live! :) (one of the happiest days!!)

Currently residing in NC and loving it!

03/2011 Looking into getting dh US citizenship (and just when I thought we were done with all the paperwork! Ha!

Timeline

That should be fine as it's on an official Ministry of Health form. He should bring that along with the updated tetanus record when he goes for his medical at the panel physician.

Should be OK. I have seen one's where nobody could figure out what name was signed so I don't think this is an issue. My immunization record has no phone number and a scribble for a name. Was it on yellow card stock or did they just give you a photocopy? BTW, who sent you this document? A doctor's office or public health or ???? The panel physician is the ultimate arbiter of whether this is acceptable or not, but I really can't see a hassle with this. Have you picked out the doctor who will do the physical? From what I've heard most immigration physicals are pretty lax. When Grandma got hers done in 1970 (Chuck Harnick's (remember him?) old man was the doctor) it was about the same. Some doctors are pretty lazy so they don't fill out all the sections completely. My parents' physician knew why they were getting the shots so he made sure that the form was filled out completely.

That is the one I used. I still had my original from when I was born. It's so paper thin! :lol:

all I had was this little pamphlet thing my mom had from when I got my vaccinations as a baby.. it didn't look very official... but when I phoned to set up my medical I asked the lady if that was ok to bring as proof and she said yes...

I ended up not showing it because all they asked for was the results from titer tests I had taken because me or my mom couldn't remember if I had the chicken pox and I didn't have any proof of getting the rubella shot (although I did remember getting it in junior high)

Looked pretty ghetto. Erm... as others have read about my non-invasive medical at Seiden's, she just asked if I had brought records and barely looked at my stuff and checked off that I had everything except for the ones that weren't age appropriate...

Misa-Yeah, the health unit woman was actually surprised they had his records too. :lol: She thought they wouldn't have them but when she checked they did.

IR5FORMUMSIE-It was the health unit that sent us the form. It wasn't on the yellow cardstock, just a plain white piece of paper. But it doesn't look like a photocopy either. It's not even the same layout as the yellow one, but it does have the Ministry of Health info on it. She said it wasn't an official form because it doesn't have the dr's signature or anything. That's one reason I was concerned about it. She said it was basically that they asked his mom which vaxes he's had, she told them and they marked it. So, I didn't know if that would pass or not. Of course, we won't mention that part of it to the dr. Oh, and we've decided on Seiden.

thetreble-Paper thin...I KNOW! :lol: This one is like that too!

liz-Hope all goes well Monday! Let us know how it goes.

Daisylynn, did it even have a seal on it? Did the health unit tell you how you could get a copy of the official report? Mom knows best I guess but I always thought they check with the family doctor/pediatrician to verify. They do in Toronto I know that.

Misa, did Dr. Seiden do the physical or was it his assistant Dr. Nick Riviera? Why did he ask you to turn your head and cough? Did you get any treatment for your ongoing case of bonus eruptus?

Liz, please let us know all the gory details of your physical with the good doctor.

Actually I think it was Dr. Riviera because now I have a leg for an arm and an arm for a leg which makes life... interesting. :P

Those will be fine. I had my girls done up recently and there was no signature or anything on them....but it was on Saskatchewan Healthcare or something official like that. No questions asked. Those ones you have are just lovely... :)

Oct 1/09 - I-751 package sent

Oct 3 - package delivered

Oct 5 - NOA mailed

Oct 13 - 1st NOA received and additional letters for my daughters I filed with. NOAs stated Resident Status

has been extended for 1 year and that we will be receiving a letter for an ASC appointment for

fingerprints, photo, and signature.

Dec 2009 - Received 10 year permanent resident card.

Feb 2010 - My children received their 10 year permanent resident cards.

Oct 2010 - My children were legally adopted by my USA Citizen husband.

Hi ya - Daisy - this is exactly what I got (we;ve PM'd about your husband an I living in the same city) I'm a couple years older, but that's the only difference. Mine also didn't have the Healthcard #, and yet my daughter's did.

Dr Seiden had no problems with it, I don't think you have anything to worry about. I had my doctor write a simple letter also stating:

- how long I had been his patient

- the current state of my health

- any current medications and the dosage (think he mentioned that I take the Pill)

Eh Voila!
