DS-156 Question - Packet 4 from MTL

I forgot to photocopy the DS156 that I sent with my packet 3. RATS!!! Can anyone who has been interviewed recently confirm whether they actually asked for copies of that form?

(Too much in a rush to get the @#$$ packet 3 back....)

Did you send in 2 copies of the DS156 in to the Consulate? If you did no new copies are needed. If there is any change in the information just let the person know and she/he will correct it using white out :)

From what I remember they asked for duplicate copies of the D156 with the barcode and I did send BOTH of those with packet 3. Maybe I should email them and ask them just in case. sheesh.

If you sent them duplicate copies of the DS-156 and one completed DS-230 you are good. They will just ask you if any information has changed at your interview if all is good they will use those forms, if there are any changes they will correct the same forms and use them.

If it makes you feel easier type out new copies before your interview and take along with you. Thats what I did but I ended up bringing all the new DS-156 and DS-230 forms back. :)

One quick question:

I received my Packet 4 from MTL on Friday, and there in amongst other requirements, there is one that says:

ALL DS-156 FORMS ARE REQUIRED IN AN ELECTRONICALLY COMPLETED FORMAT IN ENGLISH.

FORMS MAY BE DOWNLOADED AT : HTTP://EVISAFORMS.STATE.GOV

PRINT THE ENTIRE FORM INCLUDING THE BAR CODE IN DUPLICATE.

Q: There was a DS-156 form in my Packet 3 that I think it says that I need to sign this at the Interview day!

So, should I fill out that form (from my Packet-3) as well as an electronic version in duplicate ?

I am [slightly] confused? :huh:

to post an answer to my own question:

You need to take 2 copies (or three) of the DS-156 that you sent back to MTL along with your P3 checklist.

DS-156K is the single sheet of form and is the one that you will be signing at the Consulate. Do not sign until you are asked by the US Cons officer!!!!!
