i am trying to call st. luke's in manila but it said : your call could not be connected..

did i dial it correctly...011 63 2 5210020/5218647 because i am going to ask how to get another DS3025 copy from them if they going to mail it to me or email it to me... or whatever

please help me. thank you

Imaskin you how in the hell did you loose the DS-3025? You could always tell USCIS that the dog ate it. Hehehehehehe!!!!

maybe its just misplaced... st luke's was really implying to us that make a multiple copy to make sure we have back ups... if you really lost it, contact st luke's again, and ask what are the options. it could take some time to process it, but this will be the only way to prove your vac. record... good luck and God bless

the last time i had it with me was when i went to the portland clinic last week...but i knew i didn't hand it to the lady in the counter or to anyone. First of all they didn't accept me because i need to make an appointment first. But when they called me on the phone they said they will ONLY take full medical :devil: oh gosh i ONLY need the Td shot and transfer my record to I693A.

When i found a civil surgeon that will only get me a Td shot and transfer to I693A(which ate cora have recommended me because she went there)...that was the time my DS3025 was lost :angry:

I called that civil surgeon and he said that if i lost my DS3025 i will need to pay $100 and another pay for the shot.

Oh even the other copy of my original DS3025 form was lost because i put them together... :crying::crying::crying:

what i have left is the appendix C of DS3025...that is also proof that i only need my Td shot...but there was no sign of the panel physician.... :crying::crying::crying:

I wish i could tell that the dog ate it...but they had no dog...only raccoon hehe kidding

have anyone of you tried calling manila from the states? what is the right number to dial? does it have to be...011 63 2 +telephone number....is that correct? i was trying to call st. luke's yesterday but i didn't get through. thank you.
