how to package an AOS application for mother

I realize this question might have been asked many time but I have few questions regarding AOS package.

 

1. For concurrent filing can I put 130/485/ead/ap in same package or different packages within same package?

2. Should I send Separate checks for 485 filing + biometric(one for $1140 , one for $85)  or one check of $1225?

3. Do we still need to submit biographic g325 form for I-130 or no?

4. I am filing for mother, do I have a choice in selecting USCIS field office for AOS as reflected in the question 61a on page 8.

 

Anyone have cover letter for parents AOS from B2 can you please share?
