nvc keeps on rejecting my moms birth certificate

Hello guys, we have been facing an issue lately concerning my moms bc but we found a solution, so we tried uploading again to nvc but unfortunately I uploaded the wrong document. In this case I couldn't delete the sent document so I just uploaded it again under the additional documents by creating a new file (Called it birth certificate). Obviously the first submitted was rejected but the one under additional got accepted. We felt relieved and we uploaded the same document under the required documents but it got rejected once again although it got accepted under the additional documents. This is very confusing can someone please inform me why this is happening? Thanks in advance.
