I-90 | Part 3 | Omg please help

So I was literally a baby when I came into this country and we were granted Resident in 1992. I am trying to renew the permanent resident card and not even my Mother is sure what to put for Items 1- 3.a.1.

 

All I know is we left Shannon Airport in Co Galway to JFK in New York. 

In 1992 my mother had her interview in Dublin, Ireland. 

The address she gave people in NYC on our arrival was my Aunt's home in NJ. 

The Code on my magnetic strip on the Permanent Resident Card I have is an MSC code.

Yes. I am holding her passport. It says we went out of PFIShannon in Ireland in March of 1989.

 

The next stamp is the temporary evidence of lawful admission for permanent residence april 3, 1992 from the Us Immigration PFI - Shannon ..
