Question about DS260

I'm filling out the DS260 for my wife... and we have a question here.

It asks about Primary Occupation... and I have filled out her CURRENT employer.

After that it asks.

"Do you have other occupations?" Yes/No.

Is this asking about PREVIOUS employment? Or other jobs she might have on the side? She does not have any other current employment

The top of the page even says 
"NOTE: Provide the following information concerning your current employment or education."


I'm inclined to say "No"... as she only "CURRENTLY"  has that one primary occupation.

Also... For a tiny bit of extra money, my wife also clean's a friend's house, only once a week for a small amount. 500 Baht each time. Does that have to be included? It isn't a formal employer or anything. 

Additionally, my wife can't get ahold of her vaccination records. What do I put down as an "Explanation" for that particular question? She can get these Vaccines at the appointment,... correct?
