Vaccination Documentation Worksheet AOS

When it comes to sending your Vaccination Documentation Worksheet with the I-485 form (AOS) what do you do if you were not given or sent this at time of medical???

I have read a copy of it will be in the white envolope you give at POE, but can I be sure this was actually sent to UCIS and they will find it when I send all my AOS paperwork in?? 

I do not have any documentation from the Medical apart from a CD they sent me (of my scan) and a small paper explaining I paid for the flu vaccination and they administrated it there. 

I am hoping I do not have to go and have another medical now I am in the USA...

Any help would be much appreciated!

Is there any way you can contact the medical office you had your exam with? If you know the date of your medical they may be able to provide you with a copy. If USCIS lost the medical that came with the K1 and you don't have your copy, you'll have to re-do it in the US.
