DS-160 Spouse Details

Hi,

 

When filling out the DS-160, its confusing as to how to fill out spouse info.

2 Sections :

- Travelling Companion

- Spouse

 

1. My Spouse's passport is in her Maiden name so would it be correct to capture her maiden surname in the Travelling Companion section ?

2. In the Spouse section where it includes surnames (include maiden names) would it be correct to include both her married and maiden names e.g MARRIED MAIDEN ?

 

Additionally, how do you represent a hyphenated given name ? I assume instead of the hyphen, I would just use a space ?

 

Any help would greatly appreciated.
