Just an inquiry, could you please help me. My father's name was not stated in my birth certificate, should I put his name in any of the forms that I have to fill out?

In the Phils., at the time i was born, the child cannot use the father's family name unless the parents were married. As my father and mother never married, the box where my father's name left empty.

How did you answer the question on your many visa application forms?

Consistancy is usually a good policy, unless you used an incorrect answer previously and want to make a correction now. If you choose to make a correction, I recommend that you consult with an immigration lawyer first to make sure there are no unintended consequences.

Thank you for your reply...as we are just starting the process of applying for a COS, the only form that has been filled out would be the original Visa Application to inter the USA. There was no mention of fathers name on that form. So, I guess the question is, Should I write in his name on future forms or just leave them blank as it is on the birth certificate. Thank you for your help.

My son's BC does not have a father's name on it either. When we filed out his I-485 (AOS), we put NOT AVAILABLE on Father's Name.

08/17/08: Mailed N400 to TSC

08/19/08: USPS attempted delivery

08/20/08: TSC received N400

08/21/08: TSC cashed check

09/02/08: Received NOA...........Priority date: 08/20/08

..............................................Notice date : 08/22/08

09/02/08: Received Biometrics Notification

09/18/08: Biometrics completed - Charlotte DO

10/24/08: Received Interview Letter

12/08/08: Interview @ 1:00pm. APPROVED!

01/05/09: Oath Ceremony 10:00AM. Now officially a USC!!!

01/17/09: Applied for US Passport and passport card

01/28/09: Received US Passport

Oh, I'm sorry, I thought it would be in one of the biographic forms.. perhaps as a K you didn't have any forms that asked that question.

We put UNKNOWN on all of my fiance's paperwork - only his mother is noted on his BC.

11/2004 - Met in Brazil

09/2006 - Apply for K1

03/2007 - K1 approved

04/2007 - Apply for AOS & EAD

07/2007 - EAD approved

01/2008 - Conditional Residency approved

11/2009 - Apply to remove conditions

02/2010 - Permanent Residency approved

11/2010 - Apply for Citizenship

03/2011 - Citizenship approved

07/2011 - Moved back to Brazil
