@Dan 1981 - I didn't go through the IR-1 process, but looking at https://travel.state.gov/content/visas/en/immigrate/family/immediate-relative.html#10  you SHOULD have received your medical in a sealed envelope (and you hand it over, unsealed, on entry) ... My sealed medical arrived in the same package as my passport and visa.  Have you had a chance to email the Consulate and find out why it was  not sent? Hope it gets resolved quickly for you!

When You Have Your Immigrant Visa - What You Should Know

If you are issued an immigrant visa, the consular officer will give you your passport containing the immigrant visa and a sealed packet containing the documents which you provided. It is important that you do not open the sealed packet. Only the U.S. immigration official should open this packet when you enter the United States.

Oh man. How stressful. My interview is 6 June and I fly 24 June. I'm thinking surely I'll have my passport in hand by then. They express post it which is a little scary. I don't particularly trust Australia Post but what can you do!?

I had the interview on a Tuesday and got my passport back on the Friday so just a few days. There was another Aussie recently though who waited about ten business days so they could be a bit slower at the moment.
