This is your second appointment to CDJ - correct??? So you have already completed applications for CDJ that CDJ sent you in Packet 3? If so- you just need appointment letter that you just got as well as passport and bring all your proof of relationship and evidence of support stuff. Packet 4 has nothing enclosed other than letter since it will be your second appointment. If it has been more than a year since your first appointment you will need to redo the medical and pay fee again. However just to be on the safe side CDJ now has their forms on their website (new since Oct/Nov 2006) - you might just want to fill out again just in case if it has been a long time since your 1st appointment. Better to be safe. You probably won't have to turn it in but if they ask for it again at least you will have it.

8/02/06: Interview - Visa not recevied - put on AR "false hit" fingersprint check had to be done.

11/15/06: Called DHS to see if they had any info on my case - They advised security cleared in Oct. - now waiting for appt letter from CDJ for Visa Pickup. :)

11/27/06: recvd letter from CDJ - Appointment set for Jan 11, 2007

1/11/07: Visa received!!!!!

4/3/07: NOA1

4/23/07: Biometrics Appt

5/2/07: AOS transferred to CSC

5/21/07: Email from CSC stating AOS pending

6/12/07: EAD Card ordered (received email)

6/20/07: EAD Card received in mail

8/23/07: Card Production Ordered

8/30/07: Greencard received in mail

Lifting of Conditions:

5/26/09: I-751 mailed

6/1/09: NOA1

6/3/09: Check Cashed

7/7/09: Biometrics Appt

10/30/09: Approval Date

11/5/09: Received Notice in the mail

11/21/09: Greencard received in mail!

Naturalization:

8/12/10: N-400 Mailed

8/20/10: NOA1 (check cashed 8/24)

9/8/10: Early Biometrics. In and Out in 5 min!

9/22/10: Scheduled Biometrics Appt

11/4/10: Interview Appt - Passed!!!

01/27/11: Oath!!! Yippee!!

CDJ doesn't accept the visa application that they mail to you. In fact, you need to go to the Dept. of State or the CDJ website, fill it out online, then print 2 copies.

I found the applications and forms elsewhere online, but MUST it be filled out online and then printed or can I hand write it?

Yes, you must fill out online and print. See below to see what the CDJ website says. Ironically these instructions are not in the packet they sent me. This is for the visa application we're filling out for a K-3, I believe it's the same for a K-1. You can fill it out and save it as a pdf file, which can be opened with Adobe Acrobat. Then you can email it or print it and mail it the old fashion way. I DO NOT recommend USPS Air, we have a lost package. This time I'm doing it via DHL. I'm mailing it since I'm signing on the visa app. that I helped prepare the visa application.

These forms must be filled out on line, printed, signed by the applicant, and any third party who assisted. The EVAF DS-156 form contains a barcode that allows the electronic transfer of data, thus reduces the waiting time during the visa application process.

Note: If applicants fail to present the electronic forms the day of the appointment, the Consulate will not accept hand written applications and applicants will be asked to re-schedule their appointment using the

01-900-849-4949 telephone number.
