Difficult RFE reply

Hey all, posted here about a month ago regarding an RFE that we received for a multiple filing waiver,

 

Basically, the issue was that the blue sheet with our information on it had the correct names, however the actual RFE stating what we needed (A multiple filing waiver) had the complete wrong names on it (Mine and partners name is Tess and Cam, had correct address and names on mail, wrong RFE names). We contacted USICS and they have replied:

"Good morning, Please respond to the Request for Evidence by the date indicated on the blue Notice. Include a written narrative explaining the name discrepancy and why you are unable to supply some or all of the requested evidence. Your inquiry will be reviewed by the adjudications officer. We hope this information is helpful to you. Thank you, Officer Elias"

 

From my understanding what they are saying is that we need to send in a multiple filing waiver (Which we did with the application) along with something asking about the name discrepancy. Basically, my partner and I suck at writing things and could use a little help in what to write to them regarding both the multiple filing waiver and the name discrepancy. Another problem, we didn't keep the original I-129F petition number so we aren't sure that the one supplied to us pn the white RFE is the one of our application back in 2015 or not.
