Hey all, I am sorry to ask this question again but someone messaged me exactly what to do and it looks like that has disappeared out of my inbox :(

 

I was short on two vaccines when I had my medical at London Knightsbridge, 1 x MMR and 1 x Tetanus... I had these vaccines at my GP after the medical and had my K1 approved. I have been living in the USA now for 11 days and looking at the AOS process. I know that I need to see a US civil surgeon to get the transcript from them for the vaccinations I have received but do I just need to request the vaccine part of the I-693?

 

I am confused about what to do as I did have a complete instructions of what to do but for some reason it is no longer there.

 

Can anyone help me? especially anyone who has already done this?

 

I called one but they are telling me its $195 then additional costs for the test for TB and the other test that I can't spell the name.

I have already had these and do not need them but I am confused what I do here. Because this will be in my medical record that they already have... as far as I am concerned or see it is they just need proof that I have had all the vaccines required.

Take the DS3025, and any proof of the rest of your vaccinations to the Civil Surgeon that will help you complete the I-693 vaccination supplement portion ONLY.

Nothing else additional is required, except for providing some ID to the CS. No additional tests or expensive nonsense. It's just a matter of finding a CS that will do it for you. How to complete it should be on pages 6 and 7 of the instructions for the form. You fill out some stuff, they fill out the rest, they put it into an envelope and mark it.

Thanks Yuna, I think it was you who helped me a while back I just couldn't be 100% certain. I have just called a US CS who has said it will be $80 just for that vaccine part which is perfectly fine :)

I will look for the instructions on how to complete it and what I will need to fill out before they seal it up. I wanted to send it along with the packet instead of waiting for an RFE.
