My fiancé didn't have his shot record at the time of his medical in Ukraine but brought them with him to the US. Unfortunately we got them translated yesterday and I see it's been more than 10 years since he's had Tetanus, nothing was written down for Varicella and he received MPR (Measles Paratitis Rubella ) instead of MMR. Does this mean he needs to get all the vaccinations done before we can turn in AOS

If yes, we will get them done at a local clinic then go to the civil surgeon to have her transfer the records onto the I693.

If he only gets one MMR done out of the series of two can we still send in the AOS

We went to the civil surgeon first, then they said what we needed to get.  After we got the 4 or 5 vaccinations she was missing we went back and got the form.  A local clinic is a good option if you have one convenient, definitely cheaper.  The hardest one we had to find was chicken pox, most expensive too.  I would also call ahead and make sure the civil surgeon isn't going to make him go through the tuberculosis tests and the whole physical since you already had that (as long as you are within the time frame).  Some want to ding you $500 + for the exam, which you probably don't need.  I called around to about 4 before finding a good one.
