G-325A and I-130 Other Names Used

I am an F-1 Student and I got married to an American citizen. We are filling out the forms for my adjustment of status and we came across a section we were not sure about.

My wife's last name on the birth certificate initially was Blankenship. A few years later when her single mom got married, the parents then changed the last on the birth certificate to Knight. However, they didn't change it with the Social Security Office. It wasn't until recent months that my wife discovered this. Throughout her whole life she has gone by last name Knight. In the last few months we have changed her name with the Social Security Office from Blankenship to Knight, and then from Knight to my last name. Our question is, when the form asks for "all names used", would Blankenship be included in that? If we do include it, should we attach an explanation to the forms since Blankenship is not showing up on any of her documentation at the present time?
